A business system is a documented procedure that outlines how to do something in your organization to achieve your business goals.

Unfortunately, there isn't one size fits all for contact forms.  One thing for sure is that a vague contact form does not help you or the customer experience.  The name and question are a certain way to have unnecessary contact.
